Job description
Molina Healthcare is seeking a Clinical Care Manager RN in Illinois to support care management and coordinate services across the continuum. You will work with an interdisciplinary team to develop care plans, conduct assessments, and monitor progress.
The role involves travel 25-40% locally, telephonic or in-person visits, and collaboration with behavioral health and LTSS resources. Active RN license and EMR/HIPAA knowledge are required.
