Field-Based RN Case Manager: Care Coordination Leader

Job description

Oak Street Health is seeking an experienced RN, Case Manager to lead care coordination for a panel of complex patients in a value-based care setting. The role is field-based and on-site, collaborating with PCPs, social workers, and behavioral health specialists to optimize transitions of care and reduce avoidable admissions.

The ideal candidate has 6–8 years of nursing experience, an active RN license, and CCM certification (or commitment to obtain within 12 months).
