Job description
CVS Health is seeking a compassionate Registered Nurse Case Manager to join our field-based team serving Tidewater, Newport News, Hampton, and Williamsburg areas in Virginia. This role blends in-person visits with telephonic outreach to deliver coordinated, patient-centered care.
Responsibilities include comprehensive assessments, care planning, care coordination, and referrals to resources. Travel up to 75% with a Monday–Friday schedule and opportunities for a CVS Health bonus program.
