Job description
CVS Health Corporation in New York seeks a field-based Registered Nurse Case Manager to perform member assessments for NY Better Health enrollees, annual and change-in-condition assessments, and 2-3 home visits daily.
You will travel up to 75% across Suffolk and surrounding counties, document within 24 hours, and coordinate with interdisciplinary teams. Work hours Mon-Fri 8:30-5:00 EST. Benefits and comprehensive package.
