Job description
CVS Health is seeking a compassionate Registered Nurse Case Manager to join a field-based team serving Winchester, VA and surrounding areas. You will conduct comprehensive assessments, develop personalized care plans, coordinate across providers, and support members to achieve better outcomes.
This role requires an active Virginia RN license, 3+ years of clinical nursing, and willingness to travel up to 75% with mileage reimbursed.
