Job description
Adaptive Home Health in Texas is seeking a Registered Nurse (RN) Case Manager to deliver high-acuity home health care. You will own patient assessments, manage skilled visits including OASIS, and coordinate care with physicians, patients, and families.
This field-based role offers autonomy, a productivity-based pay model at 30 points/week, and the chance to advance patient access to home health care while supporting clinicians with our scheduling and charting tools.
