Job description
Adaptive Home Health is seeking a Registered Nurse Case Manager for a full-time, field-based role in Texas. You will deliver high-acuity home health care, with a focus on wound care and infusion therapy, and build plans of care through collaboration with physicians, patients, and families.
You will advocate for patients, coordinate services, and lead case management activities to improve outcomes while working with a supportive clinical and operations team.
