Job description
Beatrice Loving Heart is seeking a Support Planner to help adults with disabilities live the life they choose through person-centered planning. This field-based, professional role focuses on case management rather than direct care, supporting Maryland's Community First Choice program.
You will partner with participants and families, coordinate with providers, advocate for participants, and document all activities in LTSSMaryland, ensuring high-quality, person-centered services.
