Job description
Molina Healthcare is seeking a Registered Nurse Care Manager to support care management and coordination activities with a multidisciplinary team to ensure quality, cost-effective member care and outcomes.
You will assess members, develop care plans, conduct visits, monitor progress, and promote integration of behavioral health, LTSS, and community resources. Travel 25-40% may be required.
