Job description
Molina Healthcare is seeking a Registered Nurse care manager to support care coordination across the continuum. You will assess, plan, and implement member care, collaborating with physicians, caregivers, and the care team to achieve outcomes.
You will conduct visits, monitor care plans, and promote service integration while maintaining caseloads and meeting deadlines. Travel up to 40% is expected as required by state contracts.
