Job description
Molina Healthcare is seeking a registered nurse care manager to support care coordination across the continuum. You will assess members, develop care plans, and collaborate with physicians, caregivers, and the care team to drive outcomes. Travel locally may be required.
Ideal candidates have at least 2 years in healthcare or care management, an active RN license, and strong communication, organizational, and problem-solving skills. Proficiency with EMR and HIPAA is expected.
