Job description
Humana, Inc. is seeking a Field Care Manager 2 (Physical Health) to assess needs, develop care plans, and coordinate services for Medicaid members in Louisiana. This field-based role requires travel within designated regions and a strong focus on holistic, member-centered care.
The RN will work with diverse populations, document care activities, and partner with providers to ensure quality outcomes. A remote home office setup is supported when not conducting member visits, with standard M-F
