Job description
CVS Health is hiring a Field Case Management Coordinator in Kentucky to support case management, advocate for members, and coordinate care with providers. This field role requires 80–90% regional travel, including visits to members’ homes, schools, hospitals and clinics, with meetings at the Louisville office as needed.
A Bachelor’s or Master’s in behavioral health or related field is required, and residence in Jefferson County is essential.
