Job description
Northwell Health is seeking a compassionate Field/Home Care Registered Nurse to join our Suffolk County team. The role involves conducting comprehensive home care assessments, developing personalized care plans, and delivering skilled nursing services such as wound care, injections, and medication management.
With a Monday through Friday day shift, you will collaborate with patients, families, and physicians to coordinate care, monitor progress, document within 48 hours, and adjust interventions
