Job description
Humana, in partnership with Inclusa, is seeking a Long Term Care Functional Screen Specialist to support Wisconsin Family Care members from Vernon County. The role involves in-home assessments, using the DHS tool to determine level of care and eligibility, with travel as needed.
The position requires a health/human services degree or RN licensure in Wisconsin, LTCFS certification, and field-based work with up to 50% travel.
