Job description
Your Health Organization is seeking a Registered Nurse to join our growing primary care team. The Nurse Case Manager will visit patients in their homes and facilities, coordinate with providers, and initiate telemedicine visits as part of a comprehensive care plan.
The role covers the Lexington, West Columbia, Chapin, Newberry, and Batesburg-Leesville area with daily travel. This is a full-time, salary-based position requiring reliable transportation and flexible hours.
