About This Role
The Field Nurse in the Home Care Division at NYU Langone Health provides safe, quality, therapeutic age-appropriate nursing care using evidence-based practice. This role is ideal for a nurse who excels in clinical assessment, decision-making, and patient education while coordinating care with an interdisciplinary team.

What You'll Do
Completes a comprehensive nursing assessment including physical, functional, psychosocial, emotional, cognitive, cultural, and environmental factors.
Derives diagnoses and needs from assessment data, collaborates with patients and healthcare teams to develop individualized care plans.
Coordinates care by interacting with other healthcare team members to optimize patient outcomes and satisfaction.
Delivers direct nursing care, ongoing assessment, teaching, and training to patients and caregivers.
Identifies deficits and barriers to effective patient education and employs strategies to enhance engagement.
Maintains accurate and timely documentation reflecting assessments, diagnoses, plans of care, and evaluations.
Assists agency leadership with achieving corporate goals and participates in quality improvement initiatives.
Performs other duties as assigned.
Requirements
Education:
Bachelor's degree in Nursing required
Experience:
1-2 years in an acute care facility or home health, hospice, and long-term home health care program
Licenses & Certifications:
Current Registered Professional Nurse License in New York
Basic Life Support Certification through the American Heart Association
