About This Role
The Field Nurse in Home Care will deliver safe, therapeutic, population-specific patient care through assessment, planning, implementation, and evaluation. This role is ideal for a compassionate RN with a commitment to quality care and collaboration within a multidisciplinary healthcare team.

Schedule: Full-Time, Day shift
What You'll Do
Analyzes assessment data to determine diagnoses or issues.
Identifies expected outcomes for individualized care plans.
Develops plans that prescribe strategies to attain expected outcomes.
Implements the identified care plan.
Coordinates care delivery.
Employs strategies to promote health and a safe environment.
Evaluates progress toward attainment of outcomes.
Practices ethically using the Code of Ethics for Nurses.
Integrates evidence and research findings into practice.
Communicates effectively in various formats.
Requirements
Education: Bachelor's degree in Nursing required for employees hired after 9/1/2020
Experience: 1 - 2 years of nursing experience preferred
Licenses & Certifications: Current and valid New York State Registered Professional Nurse license
Current and valid BLS certification required
