Job description

Field Nurse Practitioner / Physician Assistant

Acute Care in the Home. Brooklyn New York | Full-Time (Exempt) | On-site, Field-Based. $165,000 to $175,000 per year, plus a sign-on bonus and comprehensive benefits. 3 x 12-hour shifts per week, with 4 consistent days off.

The Role

Some clinicians reach a point where the system they trained in, the frantic ED, the crowded inpatient unit, stops feeling like enough. You want more time, more connection, more impact. You want to solve problems at their root, not just treat the symptoms that walk through the door. This role takes your high-acuity skills out of the facility and into the home. Your patient. Their environment. Your clinical judgment. We are seeking a compassionate, experienced Nurse Practitioner (NP) or Physician Assistant (PA) to deliver high-quality acute medical care directly to patients where they live. You will have the autonomy to assess, decide, and act, backed by a dedicated team of paramedics, nurses, and physicians.

Why This Work Matters

This is not just another job. It is a chance to practice medicine in a more human way. You will serve some of the most vulnerable patients in the community, and your work directly prevents hospitalizations, reduces readmissions, and helps people heal in the place they feel most comfortable: their home.

Practice with purpose

Go beyond transactional care. Build meaningful, trusting relationships with patients and their families, and become a real part of their health journey. Top-tier compensation and benefits. A competitive salary, a meaningful sign-on bonus, and a full benefits package, because top-tier clinicians deserve top-tier rewards. True work-life balance. A 3x12 schedule gives you four full days off every week to rest, recharge, and live your life.

What You Will Do

Deliver care that follows the patient.

Perform problem-focused assessments and hands-on interventions (IV fluids, wound care, splinting, medication administration) right in the home.

No charge nurse, no transfer delays. You assess and act. Own the care plan. Order and interpret diagnostics (labs, EKGs, POCUS), and partner with referring clinicians to build holistic care plans that honor patient goals and palliative principles.

Improve outcomes that matter. Take part in quality initiatives with real, measurable impact, helping to significantly reduce readmissions and emergency department use while keeping patient satisfaction exceptionally high.

Who You Are

Licensed professional. You hold a current, unrestricted state license as a Nurse Practitioner or Physician Assistant.

Experienced clinician. You have at least 2 years (or 3,600 hours) of experience in relevant high-acuity settings such as the Emergency Department, Inpatient Medicine, or Post-Acute Care.

EMS background (highly desirable).

Experience in EMS, especially with IV access and field medication administration, is a strong plus.

Adaptable. You thrive in fast-paced, startup environments and can flex your clinical skills to meet patients where they are, across diverse home settings.

Certified. Current BLS certification required; ACLS preferred.

Mobile. You hold a valid driver's license and have the physical ability to carry equipment (up to 60 lbs) and navigate stairs.

Schedule & Location

Field-based in Brooklyn, NY and the surrounding area. Full-time, exempt, on-site. 3 x 12-hour shifts per week, with four consistent days off. Flexibility for some evening, weekend, and holiday coverage, so patients receive care when they need it most.

Benefits

Discretionary time off after 90 days, with no cap.

Medical coverage with a choice of plans.

Dental and vision.

Company-paid life insurance.

Flexible spending account and health savings account.

Continuing education reimbursement.

401(k).

A range of supplemental plans to choose from.

Sign-on bonus.

About the Team

This is a tech-enabled medical group built on a simple belief: a meaningful portion of hospital care can, and should, be delivered in the comfort of home. Partnering with health systems, payers, and provider groups, the team improves the patient experience while holding to the highest standards of clinical quality and safety, and it stands behind its results by putting its own fees at risk.

This employer is an Equal Opportunity Employer and is committed to building an inclusive environment for every member of the team.
