Job description
Adaptive Home Health, LLC is seeking an Occupational Therapist to perform in-home evaluations and provide skilled OT visits across Texas. You will develop plans focused on ADLs/IADLs, safety and independence, and recommend adaptive equipment and home modifications.
You’ll coordinate with nurses and the broader team, documenting clearly in HCHB to support patient outcomes. The role offers field-based work with a productivity-based pay model, and a schedule with Monday–Friday hours plus occasional
