nPlease Note: This PRN role primarily serves as a Physical Therapist Educator .
nOccasional field visits may be required. Candidates must complete 5-6 weeks of Full-Time Home Health training before beginning educator responsibilities or patient visits.

nnHome visits in assigned service area: Largo, St Petersburg, Pinellas Park, Seminole, and surrounding areas.
n
n nThe Home Health Field, Physical Therapist provides hands-on care in the home for a group of patients consistently using the nursing process and clinical standards and protocols. Responsibilities include :
nnnEvaluates and develops specific treatment programs for individual patients based on their needs and in accordance with the principles and practices of physical therapy.
n
nnResponsible for providing the appropriate level of clinical direction for Physical Therapy Assistants.
n
nnPlans, implements, manages and evaluates the provision of both professional and ancillary home health services (direct care and case management) to ensure that all patients need are met and quality care provided in accordance with Federal, State, JCAHO and program guidelines.
n
nnManages the activities of the multi-disciplinary team providing care to the patient.
n
nnMonitors the provision of therapeutic services through written and verbal communication with all disciplines.
n
nnDevelops, implements, and carries out a discharge plan in conjunction with the patient, family/caregiver and members of the health care team.
n
nnInterprets agency policy to patients/families.
n
nnReviews patients visit weekly and provides plan to Home Care Supervisor and scheduler, including anticipated weekend visits.
n
nnInforms scheduler of any schedule changes and communicates caseload and patient care issues to Home Care Supervisor.

n nHome Health Physical Therapist Requirements :
nnnBachelor's Degree Physical Therapy
n
nnPhysical Therapist State of Florida License
n
nn1-year Physical Therapist experience
n
nnBasic Life Support
n
nnDriver's License (Florida driver's license is required within 30 days of hire)
n
nnPreferred Master's Degree Physical Therapy
n
n nThis position requires Level II screening through the Florida Care Provider Background Screening Clearinghouse, managed by the Agency for Health Care Administration (AHCA). More information can be found at .
