Job description
Bloom Healthcare is seeking a Field-based Population Health Advanced Practice Provider to join our Texas team, serving the San Antonio and Dallas–Fort Worth areas. You will focus on gaps closure, population health data, and care coordination for home-bound patients.
Preferred candidates hold a Master’s degree (NP or PA), TX licensure, and 3+ years of clinical experience, with strong knowledge of ICD‑10, risk adjustment, and documentation best practices.
