Job description
Alignment Health is seeking a Field Remote Nurse Practitioner or Physician Assistant to support patients in Stockton, CA through the Care Anywhere program. This role blends in-home assessments with remote care to prevent hospitalizations and improve chronic-care management.
Schedule is Monday–Friday with no on-call; field visits are about 3.5 days per week and 1.5 days remote, collaborating with medical leadership to create care plans and coordinate with PCPs and specialists.
