Job description
Alignment Health seeks a Field Remote Nurse Practitioner or Physician Assistant to join the Care Anywhere (CAW) program. This role delivers in-home medical and social support to our patients, aiming to prevent hospitalizations and manage chronic conditions.
The position requires a California license, a master’s degree (NP/PA), and prior geriatric or home-care experience. Monday–Friday schedule, no weekend calls, and field remote work with local travel.
