Job description
CVS Health in Bergen County, NJ is seeking a Case Manager Registered Nurse - Field to support members in their homes and communities. This role involves assessments, care planning, and collaboration with PCPs to ensure quality, cost-effective services with up to 75% field travel.
The schedule is Monday–Friday 8am-5pm EST; required NJ Choice certification and an active NJ RN license; eligible for CVS Health bonus and comprehensive benefits.
