Molina Healthcare seeks an experienced Registered Nurse Care Manager to support LTSS members in Illinois. You will coordinate comprehensive assessments, care plans, and waiver processes while collaborating with physicians, caregivers, and the care team.
Ideal candidates have 2+ years in health care with care management or LTSS experience, are licensed as an RN, and can travel up to 40% to support member care across settings.
