Job description
Molina Healthcare is seeking a Registered Nurse care manager to coordinate LTSS across settings, collaborating with an interdisciplinary team to drive integrated member care and progress toward outcomes.
You will conduct comprehensive assessments, facilitate waiver enrollment/disenrollment, develop and monitor care plans, and help members overcome barriers to access while communicating effectively with families, providers and the care team.
