Job description
Molina Healthcare in Fairview Heights, IL is seeking a Field RN Care Manager to coordinate LTSS and support member outcomes. This role is ideal for RNs who thrive in collaborative teams and are dedicated to enhancing member care outcomes.
You will conduct comprehensive assessments, develop care plans with members and providers, and travel locally 25-40% to ensure timely services. A valid RN license is required; CCM is preferred and a BA in a health-related field may be required in some states.
