Job description
CVS Health Corporation in Jersey City, NJ is seeking a Registered Nurse Case Manager for field-based work in Hudson County and surrounding areas. You will conduct in-home assessments, evaluate medical and social needs, and coordinate care with providers and families.
This role requires an active NJ RN license, 3+ years of clinical experience, NJ Choice Certification, and willingness to travel up to 75% in your county and nearby areas. A BSN is preferred and EHR documentation is required.
