Job description
HCR Home Care in Cobleskill, NY seeks an RN Case Manager to provide skilled nursing care, teach families, and supervise the nursing team in accordance with physician orders.
This full-time role requires a New York RN license, at least one year of home care experience, and knowledge of state and federal regulations. You will coordinate patient care, develop and implement care plans, document thoroughly, and work with LPNs and home health aides to ensure safe, cost-effective outcomes.
