Job description
21st Century Home Health Services provides compassionate, patient-centered home health care across the San Francisco Bay Area. We are seeking a Full Time Registered Nurse Case Manager to oversee care coordination for in-home patients, collaborating with clinicians to reduce readmissions and improve outcomes.
The RN Case Manager will conduct home visits, develop care plans, and ensure quality patient experience as the organization expands into Marin and Sonoma counties.
