RN Case Manager – Pediatric Home Health
$70,000–$80,000/year |Monday–Friday
Company Vehicle Provided|Travel Time Included in 40hr Workweek

Are you an RN who enjoys pediatric care but is looking for a more predictable schedule and meaningful one-on-one patient relationships?
CareGivers America – NEPA is hiring a full-time RN Case Manager to support pediatric patients and families throughout Luzerne, Wyoming, and Sullivan County .
You’ll serve as a trusted clinical partner to families, coordinate care, and help children receive high-quality support from the comfort of theirhome.
Your Role as a Pediatric RN Case Manager:
Complete patient assessments, including OASIS data collection
Develop and oversee pediatric care plans with physicians and families
Supervise and support LPNs and caregivers
Coordinate care with families and interdisciplinary teams
Ensure clinical documentation is accurate, timely, and compliant
Participate in quality improvement and staff education
Promote safe, high-quality care in the home
Territories: Luzerne, Wyoming, and Sullivan County

What We're Looking For:
Active Pennsylvania RN license (Required) – BSN preferred
Active CPR Certification
1 yearof professional nursing experience preferred
Valid driver’s license
Pediatric or home care experience preferred
Strong leadership and communication skills
Comfortable working independently in the field
