Job description
CVS Health is seeking a compassionate Registered Nurse Case Manager to serve Fairfax, Sterling, Reston, Ashburn and surrounding areas of Northern Virginia. This field-based role blends in-person visits with telephonic outreach to deliver coordinated, patient-centered care.
You will conduct comprehensive assessments, develop personalized care plans, and coordinate across providers to improve health outcomes, while traveling up to 75% and maintaining high-quality documentation in multiple systems.
