Job description
CVS Health’s Aetna team is seeking a Field Social Worker in New York. You will conduct field visits, complete care management assessments, set member-centered goals, and address barriers in collaboration with the care team.
This full-time role requires travel 50-75% within Ulster County NY area, LMSW or LCSW licensed in NY, MSW degree, and strong MS Office skills. You’ll work from a dedicated home workspace and coordinate with providers to improve outcomes.
