Job description
CVS Health, including the Aetna DSNP team, is hiring a Field Social Worker to conduct home visits, complete care management assessments, and set member-centered goals. You will collaborate with providers and care teams to coordinate services for dual eligible members.
The role requires a Master of Social Work and active LMSW or LCSW in New York, plus 2+ years of medical social work or case management experience. Travel up to 75% is expected; a private work space at home is required.
