Job description
LiveWell Homecare Agency, LLC is seeking a Primary Care Nurse Practitioner with an active Illinois license and DEA certification. The successful candidate will provide outpatient primary care services to an underserved population, working 4 shifts of 10 hours each week, with no call or weekend duties.
