Home Health Links is expanding PT coverage in the San Gabriel Valley East. This role offers flexible home health visit volume, local routes, and one-on-one care for older adults at home.
Responsibilities include completing visits, communicating updates, and documenting promptly. Requires active CA license, reliable transportation, and ability to document on your own device. Strong communication with patients, families, agencies, and the HHL team is essential.
