Job description
Optum Home & Community Care, part of the Optum family, seeks an Advanced Practice Clinician (NP/PA) to perform in-home health assessments for Medicare Advantage and other plans. You will educate patients, close care gaps, and collaborate with primary care providers in a field-based role.
Flexible scheduling offers 20–40 hours per week, with 8am–8pm coverage in a seven-day window. This role emphasizes preventive care delivered at patients' homes, with no prescribing required.
