Job description
Source One Management Services, LLC is seeking Nurse Practitioners and Physician Assistants to perform in-home Health Risk Assessments for Medicare Advantage members in the Auburn, NY area. This flexible PRN role lets you set your schedule with a minimum of eight 7.5-hour days per month, with no prescribing or on-call duties.
You will complete in-home HRAs, review medical history and medications, perform basic health evaluations, and travel locally to patients within roughly 60 minutes of home
