Job description
Source One Management Services, LLC is seeking Nurse Practitioners and Physician Assistants for a flexible PRN in-home Health Risk Assessments for Medicare Advantage members. You will complete comprehensive HRAs, reviewing medical history, medications, lifestyle, and social factors, and perform basic physical and mental health evaluations.
Travel locally within your county for assessments. Active NP or PA licenses are required, excluding Pediatrics, PMHNP and Women’s Health.
