Source One Management Services, LLC is seeking Nurse Practitioners or Physician Assistants to conduct in-home Health Risk Assessments for Medicare Advantage members in Cortland, NY. This flexible PRN contractor role offers autonomy, a simple iPad-based documentation process, and malpractice insurance with collaborating physicians.
You’ll perform comprehensive HRAs, review medical histories, medications, and social factors, conduct basic health evaluations, and travel locally within the area.
