Job description
Professional Case Management is seeking RN-Case Managers in Texas to deliver in-home nursing visits for former nuclear workers with chronic illnesses. You will conduct weekly visits (1–2 hours), perform vitals, assess home safety, and coordinate care with caregivers while maintaining a flexible schedule and ensuring quality patient relationships.
The role requires an active RN license, two years of nursing experience (including one in home care), and CPR certification, with BSN preferred.
