Job description
Concierge Solutions in Florida seeks a Home Health Medical Social Worker Intern (MSW) to provide psychosocial support to patients in Assisted Living Facilities. You will identify social, emotional, financial, and environmental factors affecting well-being and outcomes.
You will collaborate with physicians and the interprofessional team to develop care plans and coordinate resources while documenting under Medicare guidelines using eCW EMR.
