UnitedHealth Group’s Optum Home & Community Care is seeking a HouseCalls Advanced Practice Clinician (Nurse Practitioner) to perform in-home health assessments for Medicare Advantage members. This part-time, 24-hour-per-week role emphasizes education, gap closure, and collaboration with PCPs to improve health outcomes.
