Job description
UnitedHealth Group, through Optum HouseCalls, is seeking an Advanced Practice Clinician (NP/PA) for part-time in-home assessments across Medicare Advantage members. You will perform comprehensive health evaluations, educate patients, and coordinate with primary care providers to close gaps in care.
Flexible scheduling with 8am-8pm hours, non-prescribing field-based work, and emphasis on education and care coordination. Excellent team support and growth opportunities are offered.
