Job description
Senior Care Solutions is seeking a compassionate Per Diem Physical Therapist to join our home health team. You will provide skilled PT services in patients' homes, develop personalized treatment plans, and collaborate with an interdisciplinary team to promote healing and independence.
Responsibilities include completing OASIS assessments, maintaining CMS-compliant documentation, and educating patients and caregivers. A Florida PT license, CPR, and reliable transportation are required.
