Job description
Source One Management Services, LLC in Batavia, NY is seeking Nurse Practitioners and Physician Assistants for flexible PRN Health Risk Assessments conducted in patients’ homes. You will complete comprehensive in-home HRAs, review medical history, medications, lifestyle and key social factors, and perform basic physical and mental health evaluations.
Travel locally within your county and nearby areas; eight 7.5-hour days per month are required.
