Job description
Source One Management Services, LLC is seeking Nurse Practitioners and Physician Assistants for flexible PRN in-home Health Risk Assessments in Willimantic, CT. You’ll assess Medicare Advantage members, review medical histories, medications, and lifestyle factors, and perform basic physical and mental health evaluations at patients’ homes.
Expect an 8-day minimum per month, competitive pay with incentives, no on-call shifts, and local travel within about 60 minutes.
