Job description
Source One Management Services, LLC is seeking Nurse Practitioners and Physician Assistants for flexible PRN in-home Health Risk Assessments. You will conduct comprehensive HRAs for Medicare Advantage members, reviewing medical history, medications, lifestyle, and social factors, and perform basic physical/mental health evaluations.
Travel locally within ~60 minutes of home, with a minimum of eight full 7.5-hour days per month.
