Job description
Aegis Healthcare is seeking a Home Health & Hospice RN Case Manager to provide exceptional care in patients' homes across the East Valley, AZ. This full-time role includes on-call responsibilities and a compensation rate of $40–$45 per hour.
You'll lead assessments, develop individualized care plans with the Interdisciplinary Group, and ensure comprehensive documentation. We offer a supportive culture and robust benefits, including PTO and retirement options.
